Care at home / After a hospital stay

When it helps

Coming home after a hospital stay.

The first weeks at home are when a little extra help matters most.

After a hospital stay, Solura Home Care can provide extra non-medical help at home, with meals, bathing, moving around safely, and rides to follow-up appointments, then scale back as strength returns.

You are not alone

Discharge day often comes faster than anyone expects. One day your loved one is in a hospital bed with nurses down the hall; the next they are home, tired, and trying to follow a new set of instructions. Families do their best, but work and distance do not pause.

What tends to help

  • A plan before discharge. Ask the hospital team what help is needed at home, which appointments are coming, and which signs should prompt a call.
  • More hours at first. The first two weeks are usually the hardest. Extra help early, then tapering off, works better than waiting for a problem.
  • A safer house. Clear the walkways, put everyday things within reach, and set up the bathroom so getting in and out is easy.
  • Rides to follow-up visits. Follow-up appointments matter, and a familiar person beside you makes them easier.

How Solura helps

Our caregivers help with personal care, meals, light housekeeping, medication reminders, and safe movement around the home, and our transportation service gets your loved one to follow-up appointments and back. We are a non-medical agency, so we work alongside the nurses and therapists in your loved one's plan rather than replacing them.

Services that often help: personal care, companionship, meals and housekeeping.

Questions

What families ask.

How quickly can care start after discharge?
Call us as soon as a discharge date is mentioned. We will give you an honest timeline and do our best to have help in place for the first day home.
Do you provide nursing care?
No. Solura provides non-medical care: help with daily activities, meals, reminders, companionship, and rides. If your loved one needs skilled nursing or therapy, we work alongside that team.
Can we reduce hours once things improve?
Yes. Many families start with more hours and scale back over a few weeks.
